The Professional Regulation Commission will conduct the practical phase on Jan. 6 to 9 and written phase on Jan. 12 to 14 of the Dentists Licensure Examination (DLE) in Baguio City.

The testing venues are University of Baguio for the practical phase and Saint Louis School, Inc.-Naguilian Road for the written phase.

All examinees are required to accomplish the entry record sent to their email addresses or posted at www.facebook.com/prc.cordillera for monitoring and contact tracing purposes.

Examinees coming outside Baguio, La Trinidad, ltogon, Sablan, Tuba, and Tublay must adhere to the latest entry protocols of Baguio City as posted at www.facebook.com/pio.baguio.

At the venue, examinees must present and submit the notice of admission and any of the following: photocopy of the complete vaccination card/certificate if fully vaccinated; hard copy of the negative result of the RT-PCR test, date of collection should be from Jan. 1 to 5, 2022; or original copy of the certificate of mandatory quarantine for 14 days that should end within Jan. 1 to 5, 2022 or its equivalent signed by any government or private physician or barangay officer visiting the examinee’s residence attesting that he/she has been in quarantine for 14 days.

The examinees must present an accomplished informed consent form and health declaration checklist downloadable at www.prc.gov.ph.

An examinee may be allowed to defer taking the licensure exam if they submitted their written request and a photocopy or electronic copy of their notice of admission on Dec. 22.

Transfer to another testing center is no longer allowed.

If the examinee cannot take the test for health-related reasons, they must submit a written request and a medical certificate on or before Jan. 21, 2022.

Those who cannot take the test due to quarantine-related travel restrictions must submit an official certificate of border/travel restrictions issued by the concerned local government unit, also on or before Jan. 21, 2022. They will be allowed to take the immediately succeeding licensure exam.

Examinees must also submit the post examination health surveillance form to [email protected] 15 days after the examination or on Jan. 29, 2022.

Examinees and examination personnel are reminded to strictly observe the minimum health standards set by the Department of Health and to comply with the provisions of PRC Memorandum Order 2020-68 dated Nov. 6.

Examinees and examination personnel are advised to use additional layer of clothes, bring rain gears, and flashlights in case of power interruption.
