The Professional Regulation Commission announced the conduct of the Physicians Licensure Examination in Baguio City on Sept. 11, 12, 18, and 19 will be at Saint Louis University Laboratory High School, Navy Base.
The PRC said if Baguio City shifts to a higher quarantine classification, the examination will be suspended.
Examinees must accomplish the entry record sent to their email addresses or posted at www.facebook.com/prc.cordillera for contract tracing purposes.
Examinees from outside Baguio and La Trinidad, ltogon, Sablan, Tuba, and Tublay, Benguet must adhere to the latest entry protocols of Baguio City, such as the presentation of Visita QTP, which is acquired by registering at https://visita.baguio.gov.ph; valid government issued ID; and negative RT-PCR test result valid within 72 hours from time of test.
Examinees must bring their Notice of Admission; ballpens and pencils; negative result of the RT-PCR test taken three to five days prior to the examination or Certificate of Quarantine or its equivalent signed by any licensed government or private physician; Municipal Health Officer, Provincial Health Officer, or City Health Officer; or barangay officer visiting the examinee’s residence attesting they were in quarantine for 14 days.
They must also bring informed Consent Form and Health Declaration Form (Annexes A and B of Memorandum Order 68 downloadable from www.prc qov.ph); face mask preferably N95 and face shield to be worn at all times; 70 percent ethyl alcohol for regular hand disinfection; and packed meal/snacks and drinks.
Examinees who fail to take the test on their schedule will be allowed to take the succeeding licensure examination subject to the following:
Those with health-related reasons must submit a letter to the Commission and the concerned Board together with a notarized medical certificate and
Those with travel restriction-related reasons must submit a letter with attached official letter or restriction from the LGU concerned.
Examinees who cannot take the examination for other reasons may submit a request to the PRC for them to take the next scheduled examination stating the reasons of their failure to take the test.
