BONTOC, Mountain Province – The municipal government continues to provide financial assistance to its indigent constituents who are undergoing medical treatment under the Bontoc LGU Assistance Program.

Municipal Social Welfare and Deve-lopment Office OIC Stephanie Kanongkong reported for this year, four individuals – three from Barangay Bontoc Ili and one from Barangay Samoki – received P10,000 each to augment their medical expenses.

Those who may avail of the program are Bontoc residents with heart illness who need to undergo surgery; chronic kidney patients who need to undergo surgery, such as kidney transplant, and need to undergo hemodialysis and peritoneal dialysis sessions for the first time; and cancer patients undergoing chemotherapy/radiation and/or who need to undergo surgery. 

The assistance will be given to qualified clients once a year. A maximum of P10,000 is given to a heart and/or a cancer patient who has a medical bill of P50,000 and above. If the medical expense is less than P50,000, the maximum assistance is P5,000. For kidney patients, the maximum financial assistance is P3,000.

The requirements must be submitted and valid within three months upon discharge from hospital. If with an unsettled bill, an itemized statement of accounts with complete name and signature of the billing clerk should be submitted. 

Required documents are medical certificate with signature and license number of attending physician not later than three months, barangay certificate of residency, voter’s certificate from the Commission on Elections, social case study report from the MSWDO for the P5,000 to P10,000 financial assistance; and certificate of indigency from the MSWDO for the P3,000 financial assistance.
